General Forms/Templates
Blank forms used in the AMMP are linked below and are also listed on CDFA's forms page. Grantees will receive project-specific copies of required forms at the beginning of the grant term or upon request for forms that are used only as needed. Grantees can always request copies of these by reaching out to cdfa.oefi_ammp_tech@cdfa.ca.gov.
STD 204 Payee Data Record Examples
After being notified of a grant award, grantees will receive instructions to complete a Payee Data Record form. This is an important first step and required before a grant agreement can be established for the awarded project. Find examples of completing this based on entity type.

This is a short factsheet with commonly asked questions about project expenses, reimbursement procedures, and project/grant duration for the grant programs administered by the Office of Agricultural Resilience and Sustainability. It is important to note that there may be additional details or requirements described in the Grant Award Procedures manual.
