Complaints

Instructions for making complaints alleging violations of any of the Unlawful Practices Sections (Food and Agricultural Code Sections 61381 – 61391). Please phone, fax, or e-mail the following information. All information on complaints is kept confidential.

  • Your name/the name of the company you are representing/address/telephone number
  • The name of the party you are alleging committed the violation/address/phone number
  • The names of any other participants if applicable
  • A description of the violation, with dates, prices, and all applicable information
  • A copy of a receipt showing the price of the dairy product if the complaint is regarding a sales below cost violation is helpful, if available.
  • A copy of a coupon if the complaint is regarding a coupon violation
  • For complaints alleging an offer to give something of value to obtain an account, provide a copy of an invoice, price schedule, and/or contract if possible.
